Privacy Request Form

Welcome to MyRoadHelp’s web portal for residents from California, Colorado, Connecticut, Montana, Oregon, Texas, Utah and Virginia to make  privacy requests, as provided under applicable policies.

If you are a subscriber or visitor to MyRoadHelp.com and you are looking to make a data access, data deletion or other privacy request under applicable law, please proceed by clicking the appropriate button on the bottom of this page.

How to Fill Out the Request From

We will take steps to verify your identity in order for us to properly respond and confirm that it is not a fraudulent request. In order to verify your identity, we will ask, at a minimum, that you provide your name, email address, and relationship to us, so that we can seek to match this information with the information existing in our systems.

Do not send MyRoadHelp any sensitive personal information to submit a request. 
For example, do not send us: your driver’s license, passport, social security card, or any other similar type of credential, as we will immediately delete it from our systems. 

If you are submitting the request for yourself, please include the following information:

1. Your name,
2. Address,
3. Phone number, and
4. Email address.

If you are requesting on behalf of someone else, you need to supply the following:

1. Your name or if you are a company submitting the request then also include the name of the company, as well as the written, signed authorization signed by the person granting you such power to act on their behalf;
2. Your relationship to the consumer;
3. Your email address; 
4. The consumer’s name;
5. The consumer's address;
6. The consumer's phone number; and
7. The consumer's email address.
